I don't like to tune a string up that far, so I tune everything a half step down and capo 3 instead. E D# G OR F# E ---note the 4th string D# ---note the 4th string A G# E D# After listening to the song and trying to figure out a way to play the intro on different parts of the fretboard, I'm pretty confident they've tuned one string up to make it simple. You're eseentially sliding a diad around and keeping a consistent beat on the 5th string. All fret numbers are relative to the capo.

[Chorus] It sounds like the acoustic guitar is strumming very similar chords and letting the piano play the changes. I think only the 5th string is changing notes on each chord. But for a single-guitar arrangement, I prefer something like this {essentially Cadd9 -> Em7 -> Dadd11):
"The medicine isn't kicking in..."
E|---0---0---0----------|
B|---2---2---4----------|
G|---4---4---4----------|
E|---0---0---0----------|
A|---0---4---2----------|
E|---X---X---X----------|
[Bridge] The acoustic part is buried in the mix, but here's the lead riff transcribed for a single guitar:
E|-------------------------------------------------------------|
B|--------0-------------0-------------0------------------------|
G|------2----2--------2----2--------2----2--------2--1-2-------|
E|----4--------4----4--------4----4--------4----0--------0-----|
A|------------------------------4-------------0----------------|
E|--0-------------2--------------------------------------------|

                                                        "The medicine..."
E|-------------------------------------------------------------|
B|--------0-------------0-------------0-------4---4---5-4------|
G|------2----2--------2----2--------2----2------4---4----------|
E|----4--------4----4--------4----4--------4-------------------|
A|------------------------------4------------------------------|
E|--0-------------2--------------------------------------------|

[Outro]
"I've tried everything, but nothing's working..."
E|------------0---------------0---------------------0--------------------------------|
B|--2---------2-----5---------5-----4---------0---------------0-----2----------------|
G|---------------------------------------------------------------------(let ring)----|
E|--------0---------------0---------------0---------------0--------------------------|
A|--0---0---0---0---4---4---4---4---2---2---2---2---2---2---2---2---0----------------|
E|-----------------------------------------------------------------------------------|
